Chief, Integrated Community Program

Chief, Integrated Community Program

We are looking for someone to lead the development of a health care system

Sunnybrook Health Sciences Centre, in Toronto, Canada, is inviting applications for its new Chief, Integrated Community Program. This is an exciting opportunity for an individual who is motivated by change, sets visionary goals, and achieves success by partnering with others. As the ideal candidate, you have a reputation for being a health system leader who is creative in your approach to problem solving and is willing to challenge ‘the way we’ve always done it’ to do something new. You personify collaboration and you are a skilled relationship builder who works exceptionally well with all stakeholders. If this sounds like you, we would like to meet you!

Sunnybrook’s Strategic Plan emphasizes the essential role the hospital plays, not only through its tertiary regional and provincial programs, but also by supporting the needs of those who live in North Toronto. A critical component of the plan is building Integrated and Sustainable Models of Care to ensure patients are able to access resources in the hospital when they need them most and transition to other levels of care efficiently and safely. The role of the new Program Chief will be to deliver on this and other aspects of the plan by partnering with services internal to the hospital and more importantly, with physicians, agencies and care providers in North Toronto.

About the Program

Sunnybrook’s Integrated Community Program encompasses a range of inpatient and ambulatory services that include Emergency, General Internal Medicine and several medical subspecialties, Family and Community Medicine, Acute Care Surgery, and Nephrology and Dialysis. By breaking down silos between hospital and home, we will provide ‘wrap around’ care for patients to ensure consistency in quality and access to services across the system of care.  In collaboration with our clinical programs and our community partners, the Chief will play a lead role in creating truly seamless care.

What will success in this role look like?

Sunnybrook has a lofty vision to invent the future of healthcare. This includes building a health system around the needs of more than 1.3 million patients who come through our doors each year, as well as extending our reach deep into our community. In this role, the successful candidate will build partnerships with care providers and agencies in the community we serve to support a truly integrated system for the delivery of care, which is less hospital-centered. The Chief will provide institutional leadership on issues related to the Integrated Community Program and, in partnership with the Operations Director, ensure effective and efficient program operations.
