Geriatrician

GERIATRICIAN

We are seeking a fourth Geriatrician to join the “Ageing Well Program” at Southlake Regional Health Centre. This is a well-integrated program that includes a team of geriatricians, geriatric psychiatrists, palliative care physicians, geriatric nurse practitioners, GEM (geriatric emergency management nurses), and geriatric outreach team members including social worker, occupational, physical therapist and a registered nurse.

 As a fellowship-trained Geriatric specialist, you will have the opportunity to support patients through various stages of their healthcare journey. You will have the opportunity to work in nurse and nurse practitioner supported clinic, in person and via OTN, provide inpatient geriatric consultations, support in home geriatric assessments with geriatric outreach team. There is also opportunity to work in surrounding retirement homes and nursing homes with NLOT (nurse led outreach team in nursing homes). Our program can access “Southlake at home program “that provides enhanced support to high risk patients discharged from the hospital.
