Trillium Health Partners (THP) is the largest community based academic health network in Ontario serving over one million residents in the communities of Mississauga, Peel Region, and West Toronto. The hospital encompasses three main sites – Credit Valley Hospital (CVH), Mississauga Hospital, and Queensway Health Centre – offering a full range of acute care hospital services, as well as a variety of community-based specialized programs. The Credit Valley Hospital provides regional services for dialysis and Oncology together with Mississauga Hospital and Queensway Health Centre. THP also maintains a satellite program at the Humber Reactivation Center for pre-rehab, palliative and long stay patients.

The Division of Rehab and Complex Continuing Care at Trillium Health Partners is seeking a full-time Locum Tenens Hospital Medicine physicians for one (1) year to join their innovative and exciting team. The division, under the Primary Care, Rehab, CCC, Palliative Care, & Senior's Services Program, are advancing the post-acute platform in new ways, including a new rotational model design, and linking with other organizations through partnerships.

The physician will provide hospital care in a variety of settings across our four sites (Credit Valley, Mississauga Site, Queensway, and Reactivation Center). The physician will work with an experienced multidisciplinary team in a robust team-based care model. The physician will share in the provision of on call coverage for the inpatients under the team’s care with associated call stipends. General remuneration is fee-for-service based. Trillium Health Partners is an affiliated academic teaching centre with the University of Toronto. The preferred candidate will have the opportunity to diversify their clinical work with participation in teaching, contribute to quality improvement and program development activities, while developing medical leadership skills. Excellent interpersonal skills and a commitment to an inter-professional care model are also essential.

This position represents an opportunity to join a group of dedicated healthcare professionals in a progressive organization. The successful candidate will be licensed in good standing by the College of Physicians and Surgeons of Ontario. The applicant will also be either a member of the College of Family Physicians of Canada or the Royal College of Physicians and Surgeons of Canada. For CCFP candidates, advanced training in Hospital Medicine or Care of Elderly is preferred but not required.
