Family Physician

Our Centre, located in rural Ontario, provides primary health, dental and allied care with a focus on health promotion and illness prevention through an inter-professional team in a collaborative practice model. Our inter-professional team supports populations at all ages and stages of life with an emphasis on those who are high risk and/or experiencing barriers to accessing services.  Our team members have a unique approach to care, engaging in a systems perspective, a focus on community health, and attention to continuous improvement.  We have a passion and commitment for innovation in providing excellent care, while creating an inclusive work environment and fostering our role as partners in creating a vibrant healthy community, where everyone can thrive. We are currently seeking a Family Physician to join our team.

Our team is uniquely comprised of Primary Care Providers, Allied Health Professionals, a Community Health Team, and a Dental Clinic.  Our intention is to provide wrap around services for our patients in our caring community.  We aim to create connections between our patients and health care providers and our community members to encourage professional and peer support networks when needed. Our physicians manage a roster of primary care patients and leverage the expertise of an entire team of health care professionals to deliver excellence in health care.  Our care plans consider the social determinants of health, alongside medical intervention.  We recognize that optimized care comes from a patient-centred approach and each patient has specific health goals and needs, and therefore may need the services of several of our professionals throughout their lives.  Each of our health care professionals uses their full scope of experience and skills to maximize health outcomes.  Electronic medical records and digital tools enhance care and timely communication between our team members. Our staff has seen first-hand that patients are best served when the whole team works together.
