At Humber River Hospital we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve. In December 2017, the Central Local Health Integration Network (LHIN), in partnership with Humber River Hospital, North York General Hospital, Southlake Regional Health Centre, Mackenzie Health and Markham Stouffville Hospital, launched an integrated, innovative model of care delivery called the Reactivation Care Centre (RCC) – A Central LHIN Hospitals Collaborative. The RCC is located in the former Humber River Hospital Finch site, which was extensively renovated for this new shared facility.

This first-of-its-kind collaboration brings five hospitals under one roof to provide specialized care for patients who no longer require acute care, but are waiting in hospital for transfer to an appropriate alternate discharge destination such as convalescent, slow stream rehabilitation, or long-term care. Each of the hospitals has its own unit which is staffed and operated as an extension of its hospital. Humber River Hospital operates one 32-bed patient care unit.

Planning is currently underway for the opening of the second Reactivation Care Centre in the Church site of Humber River Hospital that will include a 34-bed patient care unit. This program, along with our focus on comprehensive inpatient and ambulatory senior’s care, provides an exciting opportunity for physicians interested in working with this patient population.

This position would also include cross-appointment to our Wilson site where our inpatient and ambulatory senior’s program is based.

Physiatrists, General Internists AND Family Physicians

As a member of the Department of Medicine or Family Medicine, you will help provide comprehensive medical care from admission to discharge to patients at the RCC in the capacity of the Most Responsible Provider (MRP) and/or consultant to other MRPs.  The position works in collaboration with Nurse Practitioner, nursing and members of the inter-professional health care team, including enhanced Allied Health resources to deliver comprehensive care and treatment plans, assessment and  management of acute medical and aging conditions, preventing or delaying complications, and restoration of optimal functional ability through an enhanced activation program. This position includes on-call responsibilities.

The ideal candidate for this position will have experience in team-based care of seniors and possess strong interpersonal skills. Additional training in Care of the Elderly or Physical Medicine and Rehabilitation is preferred.

Applicants must hold relevant certification with the Royal College of Physicians & Surgeons of Canada or the College of Family Physicians of Canada and are a member of, or eligible for membership with, the College of Physicians & Surgeons of Ontario.
